Online Questionnaire – Web Interface Design

CHALLENGE
Create an electronic online questionnaire to replace the outdated, unreliable paper system. As a service to the mentally and chemically affected homeless population, Company X offers an introductory questionnaire so that a person in need can obtain health and shelter services. Through this questionnaire, a person is evaluated on where best to get public shelter and which health services can be offered. This system then catalogs each individual and automatically sorts them depending on their recommended services. This application will also track answers during a live interview and offer suggested services based on answers. Finally, the application will track an individual through the process and give immediate status of a person through specific criteria.

The application portion must be clear and simple enough for a typical site visitor. The admin portion must be organized such that searching and sorting groups and individuals can be done without trouble.

Screening Application

PAIN POINTS
A homeless person would have to find and visit the homeless service center in person. He or she would then need to write legibly and clearly while answering ambiguously to multiple choice questions. Applications would often be submitted unfinished. Upon submitting, there was no receipt of completion and forms could easily be lost. Forms would then need to be manually transcribed into a clunky database at considerable time and expense. This lag would often cause people to be lost in the system or slip out of contact. The database was hopelessly out of date and disorganized. Tracking individuals through the system was all but impossible.

SOLUTION
Design a clean, organized application that maintains up to the minute status of each individual and housing opportunity. Provide housing matches and health reviews for each person’s condition. Track and schedule all following interviews and notes for each person.

IDEATION
• Clean, inviting interface that is super simple to understand and process in a logical way.
• Generate a unique, easy to recall alpha-numeric code that will quickly identify each individual to aid in tracking progress.
• Make the unique ID code available to print immediately at the end of the questionnaire so the visitor will know/have it.
• Upon entering disqualifying answers, an individual will be immediate notified and sent helpful literature.
• Admin staff will be able to easily and quickly find an individual based on ID, name, social security number, contact info, application date.
• App will automatically screen out individuals based on answers and notify Admin staff of issues.
• Anyone’s status can be manually edited and updated at anytime.
• Housing and medical assignments can be tracked and provide short or long term metric statistics. Exportable to .csv file.
• All housing situations can readily be checked for up to the minute assignments.
• Waitlist displays individuals by date, status, criteria, or anyway requested.
• Allow staff permissions, security for database.
• Easily change my login password, permissions.
